Support Worker · SCHADS L2
Person-centred care delivery
What is one thing your manager could do differently to better support your work?
More notice when the roster changes — even a day helps.
The status quo
The template you cobbled together in 2023 is still your template. Half the questions don't apply anymore. You know it, the staff know it, nothing changes.
Employment Hero, Lattice, Culture Amp. $7 to $14 per user per month. Designed for companies with HR managers. You don't have one. You ARE one, and you have eight other jobs.
Good intentions are not documentation. When you need a paper trail later, an email thread is not it. Annual reviews you actually finish are the only kind that count.
How it works
Choose from 83 templates across 21 Australian industries. Questions, rating anchors, and manager guidance already inside.
Enter the employee's name and email. They get a magic link. No login. Works on their phone between shifts.
Employee fills their self-assessment. You fill yours. Meet, talk, edit. Both sign on the device in front of you.
Branded, signed, timestamped, hashed. Stored for seven years. The kind of document that holds up later.

Seven questions per role: three ratings with written 1/3/5 anchors, then reflection, growth, wellbeing and upward feedback. Inline guidance for both sides, in plain Australian English. Ready on day one.

Security
The output is a legal document about a real person's employment. It gets treated that way.
Every signed review carries a SHA-256 hash and a UTC timestamp. If the document changes, the hash stops matching — that's the point.
Signed reviews are stored and retrievable for seven years. When a dispute lands two years later, the paperwork is already there.
Employees never create accounts. They get a one-time magic link that expires — nothing for them to remember, nothing for you to reset.

Pricing
Per-seat pricing punishes you for hiring. We don't do that. Hire fifteen people next year, your bill stays the same.
FAQ
Nothing scary. We don't take a card upfront, so day 14 can't auto-charge you. Your account becomes read-only until you subscribe — past reviews and PDFs stay accessible.
No. Employees get a one-time magic link by email. They open it on their phone, fill in their self-assessment, and sign. The link expires after 14 days.
Yes — $399 a year, whatever your headcount. Per-seat pricing punishes you for hiring; we don't do that.
Yes. Duplicate any built-in template and edit it freely. Reviews already sent keep the version they were created with, so edits never rewrite history.
Both parties sign digitally, and the document records who signed, when, and from what IP. The PDF is hashed with SHA-256 and stored for seven years.
From Settings, via the Stripe billing portal — takes about a minute. Your data stays exportable as PDFs.
